three is the standard yes. it is sometimes referred to as “leaves” on the clover ☘️ the third leaf helping the other two by getting in the middle.

typically it is for kismessitudes that are not healthy but I have heard of cases where it is sometimes for bad matesprit pairings as well. there’s even some edge cases for unhealthy moirails as well.
